The tegra crtc implementation provides a custom reset hook. However,
this hook only allocates the state, initializes it with
__drm_atomic_helper_crtc_reset(), and frees the previous state. It
does not perform any hardware reset.

Since this is exactly what the atomic_create_state hook is meant to
do, minus the old state cleanup which the caller handles, convert the
implementation to use atomic_create_state with
__drm_atomic_helper_crtc_state_init() instead.

The default programming in the driver matches the "coverage" blend mode,
so add the corresponding pixel blend mode property to let userspace know
about it.

The simple KMS helpers are deprecated because they only add an
intermediate layer between drivers and atomic modesetting.

Open-code drm_simple_encoder_init() by calling drm_encoder_init()
directly and providing driver-local drm_encoder_funcs.
Also check the return value from drm_encoder_init() to avoid silent
failures.

The plane reset implementation creates a custom state subclass, but
only initializes a pristine state without resetting any hardware. This
is equivalent to what atomic_create_state expects. Convert to it.

There's a large number of vm_get_page_prot(vma->vm_flags) invocations.
Make life easier by introducing vma_get_page_prot() parameterised by the
VMA.

This also makes converting vm_get_page_prot() to vma_flags_t easier.

Also update the userland VMA tests to reflect the change.

No functional change intended.
