UDF could fail to properly write out inode on fsync(2) due to races
with WB_SYNC_NONE writeback. Several racing fsyncs could also result in
some fsync returning earlier than all metadata buffers were properly
persisted.

Fix all these issues by using new .sync_inode_metadata method which
makes sure all inode related metadata is written to disk during any
WB_SYNC_ALL writeback.

UDF uses metadata bh list attached to inode. Switch it to
generic_buffers_fsync() instead of generic_file_fsync() as we'll be
removing metadata bh handling from generic_file_fsync().

Add the setlease file_operation pointing to generic_setlease to the udf
file_operations structures. A future patch will change the default
behavior to reject lease attempts with -EINVAL when there is no
setlease file operation defined. Add generic_setlease to retain the
ability to set leases on this filesystem.

Directory iteration code was using GFP_NOFS allocations in two places.
However the code is called only under inode-&gt;i_rwsem which is generally
safe wrt reclaim. So we can do the allocations with GFP_KERNEL instead.

ll_rw_block() is not safe for the sync read path because it cannot
guarantee that submitting read IO if the buffer has been locked. We
could get false positive EIO after wait_on_buffer() if the buffer has
been locked by others. So stop using ll_rw_block(). We also switch to
new bh_readahead_batch() helper for the buffer array readahead path.

Both submit_bh() and ll_rw_block() accept a request operation type and
request flags as their first two arguments. Micro-optimize these two
functions by combining these first two arguments into a single argument.
This patch does not change the behavior of any of the modified code.
