There are a number of internal headers local to mm/ which reference
functions and data types without including the relevant headers.

mm/vma.h is a special case that intentionally does not include additional
headers, but the others are not.

This breaks tooling like clangd (which is where I noticed this), though
the build is OK due to the C files including the headers happening to
include required dependencies.

It's better to be explicit about dependencies anyway, so add the missing
includes and fix clangd as a bonus.

The early boot gigantic hugepage allocation helpers currently mix
allocation with huge_bootmem_page setup, and leave part of the
initialization flow in architecture code.

Refactor the interface to return the allocated huge page pointer and move
the huge_bootmem_page setup into the generic hugetlb code.  This makes the
architecture-specific paths focus only on finding memory, while the common
code handles node placement and early page metadata setup in one place.

This also lets powerpc benefit from memblock_reserved_mark_noinit(), which
it did not enable before.

In addition, upcoming cross-zone validation for boot-time gigantic hugetlb
reservation is common logic.  With this refactoring, that logic can stay
in the generic code instead of being duplicated in architecture-specific
paths.

alloc_gigantic_folio() allocates a folio with refcount increated and then
freeze it, convert to allocate a frozen folio to remove the atomic
operation about folio refcount, and saving atomic operation during
__update_and_free_hugetlb_folio() too.

Besides, rename hugetlb_cma_{alloc,free}_folio(), alloc_gigantic_folio()
and alloc_buddy_hugetlb_folio() with frozen which make them more
self-explanatory.

hugetlb_cma_check() was required when the ordering of
hugetlb_cma_reserve() and hugetlb_bootmem_alloc() was architecture
depended.

Since hugetlb_cma_reserve() is always called before
hugetlb_bootmem_alloc() there is no need to check whether
hugetlb_cma_reserve() was already called.

Drop unneeded hugetlb_cma_check() function.

Use order instead of struct hstate to remove huge_page_order() call from
all hugetlb folio allocation, also order_is_gigantic() is added to check

hugetlb.c contained a number of CONFIG_CMA ifdefs, and the code inside
them was large enough to merit being in its own file, so move it, cleaning
up things a bit.

Hide some direct variable access behind functions to accommodate the move.

No functional change intended.
