BH_Write_EIO is cleared in __bh_submit(), when a buffer that has been
written before is submitted for write again. That is early: it says the
error is gone at the moment we start trying to fix it, rather than when we
have.
It also loses errors. A task whose write fails sets the flag and then goes
to look at it; if another task redirties the buffer and resubmits it in
between, the submission clears the flag and the first task sees no error at
all. Neither of them is doing anything wrong.
Clear it on successful write completion instead, in the end io handlers -
the same three the rest of this series has been converting, plus gfs2's,
which already marked errors this way. Then the flag means what it says:
the last write of this buffer that finished, failed. A resubmission no
longer hides an error that has not been fixed yet, and one that has been
fixed clears the flag when the data reaches the disk.
__bh_submit() keeps setting BH_Req, which is what the rest of the tree
reads it for.

A buffer whose write failed still holds exactly the data the filesystem
asked to be written. It is the disk that is out of date, not the buffer.
Clearing BH_Uptodate says the opposite, and callers act on it:
- mark_buffer_dirty() has a WARN_ON_ONCE(!buffer_uptodate(bh)). A
filesystem that dirties the buffer again after a failed write - which is
the normal way to retry - trips it. That is the warning this series
started from.
- a buffer that is not up to date gets re-read from disk, which replaces
the data the filesystem was trying to write with the stale on-disk copy,
silently.
- the window between the write completing and the buffer being marked not
up to date is visible to anyone holding the folio lock, so the state is
not even self consistent while it lasts.
BH_Write_EIO already records the failure, and by now every place in the
tree that needs to know about it tests that flag instead: the two core
helpers in this file, adfs, exfat, ext2, ext4, fat, gfs2, jbd2, ocfs2 and
omfs, converted one filesystem at a time in the preceding patches. The
private completion handler in jbd2 was converted along with its waiters,
and ext4 fast commit needed only its waiter, because commit 7f0485dd3017
("ext4: remove ext4_end_buffer_io_sync()") had already dropped its handler
in favour of bh_end_write(). Nothing is left that reads BH_Uptodate to
find out whether a write failed.
Setting BH_Uptodate on success goes too. A buffer has to be up to date
before it can be written - you cannot write out data you do not have - so
the only thing that assignment could do is paper over a caller that got
that wrong. Write completion now leaves BH_Uptodate alone in both
directions.
What this changes for readers. A buffer whose write failed stays up to
date, so the read paths stop replacing it with the on-disk copy:
__bread_gfp() no longer sends it to __bread_slow(), and
bh_uptodate_or_lock() reports it as usable. That is the intent. ocfs2
changes the most, because ocfs2_read_blocks() decides whether to go to disk
on its own cluster uptodate cache and only tests BH_Uptodate after the
wait, so a block whose write failed makes that read return -EIO today and
from here it succeeds and hands back the in-memory data. A caller that
needs to know the write failed asks BH_Write_EIO.

Both places in this file that report a metadata write error to a caller do
it by testing !buffer_uptodate() after waiting for the write. That works
only because the write completion handlers clear BH_Uptodate when the write
fails, which is what this series is removing: a buffer whose write failed
still holds the correct data, and saying otherwise makes callers rewrite,
re-read or WARN over a buffer that was never wrong.
BH_Write_EIO is the flag that actually means "the last write of this buffer
failed", and both handlers already set it via mark_buffer_write_io_error().
Test that instead.
No behaviour change: today a failed write through bh_end_write() or
bh_end_async_write() sets BH_Write_EIO and clears BH_Uptodate together, so
the two tests agree. They stop agreeing at the end of the series, and this
one stays right.

discard_buffer() strips the state that describes where a buffer lives and
what has happened to it, because after an invalidate none of it applies any
more. BH_Write_EIO belongs in that set for the same reason: it describes a
write of the data that is being thrown away.
Leaving it set means a buffer_head reused for a different block starts life
carrying somebody else's write error. Like the bforget() change, this is
mostly theoretical today and becomes load bearing once the rest of the
series makes BH_Write_EIO the report of a failed metadata write.

BH_Write_EIO records that the last write of this buffer failed. It is
cleared when the buffer is written again, but a filesystem freeing a
metadata block never writes it again. It calls bforget() and hands the
block back to the allocator, so the flag outlives the block it refers to.
That does not matter much today, because the write error is also recorded
by clearing BH_Uptodate and the buffer is discarded soon after. It starts
to matter in the rest of this series, which stops clearing BH_Uptodate on
write error and makes BH_Write_EIO the way a failed metadata write is
reported.
bforget() is where a filesystem says it no longer cares about this
buffer's contents, so clear the error there alongside the dirty flag.

folio_mapping() was doing two jobs here. One was to turn a slab folio into
NULL, which is what made this safe for jbd2's shadow buffers; the previous
patch removed the need for that by giving those buffers no folio at all.
The other is a hazard. folio_mapping() maps a folio in the swap cache to
its swap_address_space, so if a buffer_head were ever attached to such a
folio this would hand fscrypt a swap mapping and dereference ->host on it.
There is no reason to want that here: this path wants the file's mapping or
nothing.
Read ->mapping directly. Buffers with no folio are already handled above.

jbd2 builds a temporary buffer_head to write out the frozen copy of a
metadata block, and that copy lives in slab memory. Today jbd2 points the
temporary buffer at the slab folio backing it. A slab folio's ->mapping is
not an address_space, so anything that follows bh->b_folio->mapping there
gets garbage rather than NULL; mark_buffer_write_io_error() does exactly
that, and we are about to start calling it on this buffer.
Rather than teach every such helper about slab folios, allow bh->b_folio to
be NULL and let b_data point straight at the memory. Code that needs the
folio has to check. There are two places in this file:
- __bh_submit() adds the data by virtual address using
bio_add_virt_nofail(), and skips the cgroup accounting: a buffer that is
not in the page cache has no owning folio to attribute writeback to.
- buffer_set_crypto_ctx() returns early. fscrypt has no interest in a
buffer that is not part of a file mapping, which is why it already
returns when the folio has no mapping.
Nothing sets b_folio to NULL yet, so this patch is a no-op on its own.
A buffer_head without a folio is a narrow thing, not a new general
capability. Most of the buffer_head API assumes a folio and will fault or
corrupt state without one - touch_buffer(), bh_offset(), the async read
completion path, and plenty more - so it is up to whoever builds such a
buffer to keep it away from all of that. What NULL buys us is that getting
it wrong fails loudly instead of quietly following a slab folio's
overloaded ->mapping. It is also only valid over memory that is always
mapped: buffers over highmem have no permanent kernel virtual address,
which is why folio_set_bh() records a folio and an offset instead.

Nothing in suspend.h needs swap.h. However, many files indirectly
depend on some of swap.h's dependencies, so this is a large
cross-subsystem patch. Stats:

Pull fscrypt updates from Eric Biggers:
"The main change this cycle is a significant simplification that's been
overdue for a while now: standardizing on a single file contents
encryption implementation in ext4 and f2fs, instead of having two.
Specifically, the original filesystem-layer file contents encryption
implementation is removed, and the blk-crypto implementation is now
used unconditionally. blk-crypto delegates either to inline crypto
hardware or to the CPU via blk-crypto-fallback. The latter is
functionally equivalent to the original filesystem-layer code.
The blk-crypto implementation already existed, but previously it was
used only when the filesystem was mounted with "-o inlinecrypt". Now,
"-o inlinecrypt" just selects whether inline crypto hardware is used.
To allow maintaining that user control over hardware use, the
blk-crypto API is extended with a new flag BLK_CRYPTO_CFG_ALLOW_HW.
Overall, this removes quite a bit of redundant code from ext4, f2fs,
and fs/crypto/. It should make things easier for ongoing filesystem
efforts such as iomap support, large folios, and btrfs encryption
(btrfs had already been planning to use blk-crypto exclusively.)
There are two small behavior changes of note:
- Direct I/O now works on encrypted files even without "-o inlinecrypt",
rather than falling back to buffered I/O. This is effectively a
bugfix, though I'll continue to keep an eye out for any user that
may have been depending on the buffered I/O fallback.
- IV_INO_LBLK_32 policies are no longer supported in certain cases
that didn't make sense and have no known uses.
This has been in linux-next since July 22 with no reported issues. All
encryption xfstests pass on ext4 and f2fs. As usual I've also been
using it on a system with an fscrypt-encrypted home directory. Of
course, the blk-crypto code paths also aren't new and were already
being used on many systems via the inlinecrypt mount option.
In addition to the main change described above, there are a few other
cleanups such as using lock guards for mutexes, improving
documentation, and removing a workaround for outdated gcc versions"

Set BIO_COMPLETE_IN_TASK in __bh_submit() for write bios when the folio
has dropbehind set, so that buffer_head writeback completions get
deferred to task context where folio_end_dropbehind() can safely
invalidate folios.
Read completions are not deferred since dropbehind invalidation for
reads is handled synchronously by the reader.

When mmb_fsync_noflush() or simple_fsync_noflush() race with another
writeback of the same inode, they can see inode dirty bits are already
clear and skip inode writeback although the racing
__writeback_single_inode() didn't yet get to writing anything. This can
result in fsync(2) returning without properly persisting the inode.
We already have I_SYNC bit for this synchronization and
writeback_single_inode() properly uses it so just fix
mmb_fsync_noflush() and simple_fsync_noflush() to take it into account
as well.

When filesystem is freeing inode it calls mmb_invalidate() which removes
bhs from inode's metadata bh tracking and clears b_mmb for them. However
if the inode is getting deleted, we don't bother with calling mmb_sync()
before and thus these buffers can be under IO and we can be racing with
IO completion handler calling mark_buffer_write_io_error(). This race
can lead to mark_buffer_write_io_error() either hitting NULL pointer
reference or trying to operate on already freed inode. Protect the
mapping handling with RCU to make sure mmb and inode aren't freed before
we are done with them.

block_commit_write() always walks every buffer_head attached to the
folio. That was cheap for order-0 folios, but large folios can contain
hundreds of buffer_heads. For a small buffered overwrite of an
already-uptodate large folio, the commit work is therefore proportional
to the folio size rather than the copied range.
This became visible with ext4 regular-file large folios, where cached
small overwrites reach block_commit_write() through block_write_end().
Before ext4 enabled large folios for regular files, this path was only
hit with order-0 folios for normal ext4 buffered writes, so the full walk
was bounded. The ext4 large-folio commit is therefore the regression
point for this generic helper cost.
The full walk is still needed when the folio is not uptodate, because
block_commit_write() uses per-buffer uptodate state to decide whether
the whole folio can be marked uptodate. Keep those folios on the old
full-buffer path.
For a folio that was already uptodate on entry, the commit no longer
needs tail buffers for folio-uptodate discovery. The copied range has
already been processed once block_start reaches @to, so stop there and
avoid the suffix walk.

Now that fscrypt's file contents en/decryption is always implemented
using blk-crypto when the filesystem is block-based, the fs-layer

"Features:
- Reduce pipe->mutex contention by pre-allocating pages outside the
lock in anon_pipe_write().
anon_pipe_write() called alloc_page() once per page while holding
pipe->mutex. The allocation can sleep doing direct reclaim and runs
memcg charging, which extends the critical section and stalls any
concurrent reader on the same mutex. Now up to 8 pages are
pre-allocated before the mutex is taken, leftovers are recycled
into the per-pipe tmp_page[] cache before unlock, and any remainder
is released after unlock, keeping the allocator out of the critical
section on both sides. On a writers x readers sweep with 64KB
writes against a 1 MB pipe throughput improves 6-28% and average
write latency drops 5-22%; under memory pressure - when the cost of
holding the mutex across reclaim is highest - throughput improves
21-48% and latency drops 17-33%. The microbenchmark is added to
selftests.
- uaccess/sockptr: fix the ignored_trailing logic in
copy_struct_to_user() to behave as documented and the usize check
in copy_struct_from_sockptr() for user pointers, and add
copy_struct_{from,to}_bounce_buffer() and copy_struct_to_sockptr()
helpers for upcoming users (IPPROTO_SMBDIRECT, IPPROTO_QUIC).
- bpf: add a sleepable bpf_real_inode() kfunc that resolves the real
inode backing a dentry via d_real_inode(). On overlayfs the inode
attached to the dentry doesn't carry the underlying device
information; this is used by the filesystem restriction BPF program
that was merged into systemd.
- docs: add guidelines for submitting new filesystems, motivated by
the maintenance burden abandoned and untestable filesystems impose
on VFS developers, blocking infrastructure work like folio
conversions and iomap migration.
Fixes:
- libfs: set SB_I_NOEXEC and SB_I_NODEV by default in init_pseudo()
and drop the now-redundant assignments in callers. This began as a
one-line dma-buf fix for a path_noexec() warning; a pseudo
filesystem has no reason not to set SB_I_NOEXEC. All init_pseudo()
callers were audited: the only visible effect is on dma-buf where
SB_I_NOEXEC silences the warning.
- Handle set_blocksize() failures in legacy filesystems (bfs, hpfs,
qnx4, jfs, befs, affs, isofs, minix, ntfs3, omfs). Mounting a
device with a sector size > PAGE_SIZE crashed roughly half of them;
the rest had the same missing error handling pattern. Plus a
follow-up releasing the superblock buffer_head when setting the
minix v3 block size fails.
- mount: honour SB_NOUSER in the new mount API.
- fs/fcntl: fix a SOFTIRQ-unsafe lock order in fasync signaling by
switching the process-group paths of send_sigio() and send_sigurg()
from read_lock(&tasklist_lock) to RCU, matching the single-PID
path.
- vfs: add an FS_USERNS_DELEGATABLE flag and set it for NFS, fixing
delegated NFS mounts (fsopen() in a container with the mount
performed by a privileged daemon) that broke when non-init
s_user_ns was tied to FS_USERNS_MOUNT.
- selftests/namespaces: fix a hang in nsid_test where an unreaped
grandchild kept the TAP pipe write-end open, a waitpid(-1) race in
listns_efault_test, and a false FAIL on kernels without listns()
where the tests should SKIP.
- filelock: fix the break_lease() stub signature for
CONFIG_FILE_LOCKING=n.
- init/initramfs_test: wait for the async initramfs unpacking before
running; the test and do_populate_rootfs() share the parser state.
- fs/coredump: reduce redundant log noise in
validate_coredump_safety().
- iomap: pass the correct length to fserror_report_io() in
__iomap_write_begin().
- backing-file: fix the backing_file_open() kerneldoc.
Cleanups:
- initramfs: refactor the cpio hex header parsing to use hex2bin()
instead of the hand-rolled simple_strntoul() which is reverted, and
extend the initramfs KUnit tests to cover header fields with 0x
prefixes.
- Replace __get_free_pages() and friends with kmalloc()/kzalloc()
across quota, proc, ocfs2/dlm, nilfs2, nfs, nfsd, libfs, jfs, jbd2,
isofs, fuse, select, namespace, configfs, binfmt_misc, bfs, and the
do_mounts init code - part of the larger work of replacing page
allocator calls with kmalloc().
- Use clear_and_wake_up_bit() in unlock_buffer() and
journal_end_buffer_io_sync() instead of open-coding the sequence.
- Drop unused VFS exports: unexport drop_super_exclusive(), remove
start_removing_user_path_at(), and fold __start_removing_path()
into start_removing_path().
- fs/read_write: narrow the __kernel_write() export with
EXPORT_SYMBOL_FOR_MODULES().
- vfs: uapi: retire octal and hex constants in favor of (1 << n) for
the O_ flags. Finding a free bit for a new flag across the
architectures was needlessly hard with the mixed bases.
- dcache: add extra sanity checks of dead dentries in dentry_free()

by requiring the caller put the refcount on the buffer. The only caller
is in the gfs2_meta_read() path, and there we can put the refcount

This shrinks buffer_head by 8 bytes, letting us pack more buffer heads
per slab. With a Debian config, it shrinks from 104 bytes to 96 bytes
which is 42 objects per 4KiB page rather than 39, a 7% reduction in the
amount of memory used.

Use `clear_and_wake_up_bit()` in `unlock_buffer()`, since the helper was
introduced in 'commit 8236b0ae31c83 ("bdi: wake up concurrent
wb_shutdown() callers.")' as a generic way of doing the same sequence of
operations:
clear_bit_unlock();
smp_mb__after_atomic();
wake_up_bit();
The helper was implemented to avoid bugs caused by forgetting to call
`wake_up_bit()` after `clear_bit_unlock()`.
Since `unlock_buffer()` predates git and was last modified in
'commit 4e857c58efeb9 ("arch: Mass conversion of smp_mb__*()")', years
before `clear_and_wake_up_bit()`, it still uses the open-coded sequence.
Replace the open-coded sequence with the helper to avoid duplicate code
and reduce code paths to maintain.

When a metadata block is referenced by multiple inodes and tracked by
metadata bh infrastructure (which is forbidden and generally indicates
filesystem corruption), it can happen that mmb_mark_buffer_dirty() is
called for two different mmb structures in parallel. This can lead to a
corruption of mmb linked list. Handle that situation gracefully (at
least from mmb POV) by serializing on setting bh->b_mmb.

In the near future, a folio will no longer pin its corresponding memory
cgroup. To ensure safety, it will only be appropriate to hold the rcu
read lock or acquire a reference to the memory cgroup returned by
folio_memcg(), thereby preventing it from being released.
In the current patch, the function get_mem_cgroup_from_folio() is employed
to safeguard against the release of the memory cgroup. This serves as a
preparatory measure for the reparenting of the LRU pages.

struct pagevec was removed in commit 1e0877d58b1e ("mm: remove struct
pagevec"). Rename include/linux/pagevec.h to reflect reality and update
includes tree-wide. Add the new filename to MAINTAINERS explicitly, as it
no longer matches the "include/linux/page[-_]*" pattern in MEMORY
MANAGEMENT - CORE.

Make buffer heads point to mapping_metadata_bhs instead of struct
address_space. This makes the code more self contained. For the (only)
case of IO error handling where we really need to reach struct
address_space add a pointer to the mapping from mapping_metadata_bhs.

Instead of tracking metadata bhs for a mapping using i_private_list and
i_private_lock create a dedicated mapping_metadata_bhs struct for it.
So far this struct is embedded in address_space but that will be
switched for per-fs private inode parts later in the series. This also
changes the locking from bdev mapping's i_private_lock to a new lock
embedded in mapping_metadata_bhs to untangle the i_private_lock locking
for maintaining lists of metadata bhs and the locking for looking up /
reclaiming bdev's buffer heads. The locking in remove_assoc_map() gets
more complex due to this but overall this looks like a reasonable
tradeoff.

There's only single caller of fsync_buffers_list() so untangle the code
a bit by folding fsync_buffers_list() into sync_mapping_buffers(). Also
merge the comments and update them to reflect current state of code.

The function only waits for already locked buffers in the list of
metadata bhs. fsync_buffers_list() has just waited for all outstanding
IO on buffers so this isn't adding anything useful. Comment in front of
fsync_buffers_list() mentions concerns about buffers being moved out
from tmp list back to mappings i_private_list but these days
mark_buffer_dirty_inode() doesn't touch inodes with b_assoc_map set so
that cannot happen. Just delete the stale code.

All filesystem using generic metadata bh tracking are using bdev mapping
as a backing for these bhs. Stop using i_private_data for it and get to
bdev mapping directly.
