Add __must_hold() attribute to raw3215_make_room() in order to let
clang's context analysis know that "get_ccwdev_lock(raw->cdev)" must be
held on function entry. This can also be easily verified when inspecting
the function.
Without this annotation this leads to a valid warning when context
analysis is enabled:

Since [1] cond_resched() is a no-op on s390. Remove all calls.
This also entirely removes uv_call_sched() and replaces all call sites
with uv_call(), since both functions are identical after the removal
of cond_resched().

When data buffers are reused, e.g. for interval sample records, the
first record determines the data length, and the size of the buffer for
user copy. Current monwriter code does not check if the data length was
changed for subsequent records, which also would never happen for valid
user programs.
However, a malicious user could change the data length, resulting in out
of bounds user copy to the kernel buffer, and memory corruption. By
default, the monwriter misc device is created with root-only permissions,
so practical impact is typically low.
Fix this by checking for changed data length and rejecting such records.

Pull kvm updates from Paolo Bonzini:
"arm64:
This is a bit of an odd merge window on the KVM/arm64 front. There
is absolutely no new feature in the pull request. It is purely
fixes, because it is simply becoming too hard to review new stuff
when so many AI-fuelled fixes hit the list.
- Significant cleanup of the vgic-v5 PPI support which was merged in
7.1. This makes the code more maintainable, and squashes a couple
of bugs in the meantime
- Set of fixes for the handling of the MMU in an NV context,
particularly VNCR-triggered faults. S1POE support is fixed as well
- Large set of pKVM fixes, mostly addressing recurring issues around
hypervisor tracking of donated pages in obscure cases where the
donation could fail and leave things in a bizarre state
- Fixes for the so-called "lazy vgic init", which resulted in
sleeping operations in non-preemptible sections. This turned out to
be far more invasive than initially expected..
- Reduce the overhead of L1/L2 context switch by not touching the FP
registers
- Fix the way non-implemented page sizes are dealt with when a guest
insist on using them for S2 translation
- The usual set of low-impact fixes and cleanups all over the map
Loongarch:
- On a request for lazy FPU load, load all FPU state that the VM
supports instead of enabling only the part (FPU, LSX or LASX) that
caused the FPU load request

- Do not assume that all page tables start with U=1/W=1/NX=0 at the
root, as AMD GMET needs to have U=0 at the root
- Introduce common assembly macros for use within Intel and AMD
vendor-specific vmentry code. This touches the SPEC_CTRL handling,
which is now entirely done in assembly for Intel (by reusing the
AMD code that already existed), and register save/restore which
uses some macro magic to compute the offsets in the struct. Both of
these are preparatory changes for upcoming APX support
- Clean up KVM's register tracking and storage, primarily to prepare
for APX support, which expands the maximum number of GPRs from 16
to 32
- Keep a single copy of the PDPTRs rather than two, since
architecturally there is just one
- Handle EXIT_FASTPATH_EXIT_USERSPACE in vendor code to ensure vendor
code gets a chance to handle things like reaping the PML buffer
- Update KVM's view of PV async enabling if and only if the MSR write
fully succeeds
- Fix a variety of issues where the emulator doesn't honor
guest-debug state, and clean up related code along the way
- Synthesize EPT Violation and #NPF "error code" bits when injecting
faults into L1 that didn't originate in hardware (in which case the
VMCS/VMCB doesn't hold relevant information)
- Add support for virtualizing (well, emulating) AMD's flavor of
CPL>0 CPUID faulting
- Clean up the GPR APIs so that KVM's use of "raw" is consistent, and
fix a variety of minor bugs along the way
- Fix an OOB memory access due to not checking the VP ID when
handling a Hyper-V PV TLB flush for L2
- Fix a bug in the mediated PMU's handling of fixed counters that
allowed the guest to bypass the PMU event filter
- Allow userspace to return EAGAIN when handling SNP and TDX
hypercalls, so the KVM can forward a "retry" status code to the
guest, and reserve all unused error codes for future usage
- Overhaul the TDP MMU => S-EPT code to move as much S-EPT specific
logic as possible into the TDX code, and to funnel (almost) all
S-EPT updates into a single chokepoint. The motivation is largely
to prepare for upcoming Dynamic PAMT support, but the cleanups are
nice to have on their own
- Plug a hole in shadow page table handling, where KVM fails to
recursively zap nested EPT/NPT shadow page tables when the nested
hypervisor tears down its own EPT/NPT page tables from the bottom
up
x86 (Intel):
- Support for nested MBEC (Mode-Based Execute Control), see above in
the generic section; also run with MBEC enabled even for non-nested
mode
- Use the kernel's "enum pg_level" in the TDX APIs instead of the
TDX-Module's level definitions (which are 0-based)
- Rework the TDX memory APIs to not require/assume that guest memory
is backed by "struct page" (in prepartion for guest_memfd hugepage
support)
- Fix a largely benign bug where KVM TDX would incorrectly state it
could emulate several x2APIC MSRs
- Use the "safe" WRMSR API when proxying LBR MSR writes as the
to-be-written value is guest controlled and completely unvalidated
x86 (AMD):
- Support for nested GMET (Guest Mode Execution Trap), see above in
the generic section; also run with GMET enabled even for non-nested
mode
- Fixes and minor cleanups to GHCB handling, on top of the earlier
work already merged into 7.1-rc
- Ensure KVM's copy of CR0 and CR3 are up-to-date prior to invoking
fastpath handlers
- Add support for virtualizing gPAT (KVM previously just used L1's

- Fix goofs where KVM mishandles side effects (e.g. single-step and

- Rename invalidate_begin() to invalidate_start() throughout KVM to
follow the kernel's nomenclature, e.g. for mmu_notifiers

guest_memfd:
- Return -EEXIST instead of -EINVAL if userspace attempts to bind a
gmem range to multiple memslots, and fix the test that was supposed
to ensure KVM returns -EEXIST
- Treat memslot binding offsets and sizes as unsigned values to fix a
bug where KVM interprets a large "offset + size" as a negative
value and allows a nonsensical offset
- Use the inode number instead of the page offset for the NUMA
interleaving index to fix a bug where the effective index would
jump by two for consecutive pages (the caller also adds in the page
offset)
Selftests:
- Randomize the dirty log test's delay when reaping the bitmap on the
first pass, as always waiting only 1ms hid a KVM RISC-V bug as the
test reaped the bitmap before KVM could build up enough state to
hit the bug

con3270_alloc_view() allocates a staging buffer used to assemble
3270 datastream content before it is copied into channel program
requests.
This buffer can be allocated with kmalloc() as there's nothing special
about it to go directly to the page allocator.
kmalloc() provides a better API that does not require ugly casts and
kfree() does not need to know the size of the freed object.
Performance difference between kmalloc() and __get_free_pages() is not
measurable as both allocators take an object/page from a per-CPU list for
fast path allocations.
For the slow path the performance is anyway determined by the amount of
reclaim involved rather than by what allocator is used.
Replace use of __get_free_page() with kmalloc() and free_page() with
kfree().

Rename the memory block lookup helper to make the acquired reference
explicit, add memory_block_put() to wrap put_device(), remove
find_memory_block(), and use memory_block_get() as the single block-id
based lookup interface.
This makes it clearer to callers that a successful lookup holds a
reference that must be dropped, reducing the chance of forgetting the
matching put and leaking the memory block device reference.

Remove the SCLP_OFB Kconfig option and enable the guarded code
unconditionally. This guards only a few lines of code, so the impact is
very low while at the same time this reduces the large number of Kconfig
options.

The create_class() api is retiring in favor of class_register() (see:
https://lore.kernel.org/all/2023040244-duffel-pushpin-f738@gregkh/).
The HMCDRV_DEV_CLASS define is hiding a use of create_class(), but it is
permanently disabled as it is commented out. To avoid supporting code
that is disabled, the suggestion is to remove all code hiding be behind
any #ifdef HMCDRV_DEV_CLASS.

This converts some of the visually simpler cases that have been split
over multiple lines. I only did the ones that are easy to verify the
resulting diff by having just that final GFP_KERNEL argument on the next
line.
Somebody should probably do a proper coccinelle script for this, but for
me the trivial script actually resulted in an assertion failure in the
middle of the script. I probably had made it a bit _too_ trivial.
So after fighting that far a while I decided to just do some of the
syntactically simpler cases with variations of the previous 'sed'
scripts.
The more syntactically complex multi-line cases would mostly really want
whitespace cleanup anyway.

This was done entirely with mindless brute force, using
git grep -l '\<k[vmz]*alloc_objs*(.*, GFP_KERNEL)' |
xargs sed -i 's/\(alloc_objs*(.*\), GFP_KERNEL)/\1)/'
to convert the new alloc_obj() users that had a simple GFP_KERNEL
argument to just drop that argument.
Note that due to the extreme simplicity of the scripting, any slightly
more complex cases spread over multiple lines would not be triggered:
they definitely exist, but this covers the vast bulk of the cases, and
the resulting diff is also then easier to check automatically.
For the same reason the 'flex' versions will be done as a separate
conversion.

This is the result of running the Coccinelle script from
scripts/coccinelle/api/kmalloc_objs.cocci. The script is designed to
avoid scalar types (which need careful case-by-case checking), and
instead replace kmalloc-family calls that allocate struct or union
object instances:
Single allocations: kmalloc(sizeof(TYPE), ...)
are replaced with: kmalloc_obj(TYPE, ...)
Array allocations: kmalloc_array(COUNT, sizeof(TYPE), ...)
are replaced with: kmalloc_objs(TYPE, COUNT, ...)
Flex array allocations: kmalloc(struct_size(PTR, FAM, COUNT), ...)
are replaced with: kmalloc_flex(*PTR, FAM, COUNT, ...)
(where TYPE may also be *VAR)
The resulting allocations no longer return "void *", instead returning
"TYPE *".

Recent cleanups and code consolidations in the s390 tape device driver
renamed files and function namespaces from tape_34xx to tape_3490 to
better reflect the single support of the IBM 3490E device in the
codebase.
These changes also renamed the driver name to tape_3490, which
consequently broke userspace as the sysfs driver path is now
/sys/bus/ccw/drivers/tape_3490/ instead of
/sys/bus/ccw/drivers/tape_34xx/.
Change the device driver name back to tape_34xx to fix userspace.

The tape device driver only supports 3490E devices on Virtual Tape Server
(VTS). There is no point in keeping separated options and modules for
different device types and general hardware support. Consolidate the tape
config option into a single option, which enables the complete tape support
with one singular module called 'tape_s390'. The corresponding module entry
points are adapted and consolidate in tape_init() and tape_exit()
respectively in tape_core.c
The current module author and descriptions of the individual tape
modules are outdated and haven't been changed for quite some time.
Change it to a more generic description that is in line with the
corresponding supported s390 architecture for the single tape module.

The driver now exclusively supports 3490 tape devices, given support for
3480 tape devices has been removed. Update the device driver name, its
source file name, and change any occurrences of "34xx/34XX" to "3490" in
the source code and comments.

Quite a few Error Recovery Action (ERA) codes and sense data entries are
not relevant anymore for the Virtual Tape Server (VTS) and are not being
used by VTS. Most of them were relevant for actual physical errors when
a tape cartridge got stuck or a tape didn't rewind properly for example.
Remove these codes from the sense data analysis as it's dead code
anyway.

The only supported device type by the Virtual Tape Server is 3490. The
3480 device type was an old physical tape model and doesn't exist
anymore. Remove 3480 from the list and any mention of it.

For real 3490 tape models a logical block was composed of a direction
bit (wrap), a segment number, a format mode, and a logical block number.
This is represented in a 4byte identifier.
The Virtual Tape Server (VTS) emulates 3490 tape devices and uses a
stripped block id format where bit 0-9 of the 4byte identifier are
always 0. Bit 10-31 represent the logical block number.
All tapes use the 3480-2 XF format, which was defined via
TAPE34XX_FMT_3480_2_XF but never used. There is also no special handling
required anymore as this is the only format being used.
Since VTS doesn't require any special handling of block ids and format,
corresponding code is removed.

The LOAD_DISPLAY (LDD) X'9F' is still accepted by the Virtual Tape
Server (VTS) but does not perform any action.
Remove all functions and definitions related to this command.
The tape_34xx_ioctl() function is also removed as it was mainly used to
handle additional ioctl functionality. LOAD_DISPLAY was the only left
case. All other ioctls are handled in tapechar_ioctl().
With LOAD_DISPLAY, the remaining definitions in asm/tape390.h are gone.
Delete the file.

Physical 3590/3592 tape models are not supported anymore for a very long
time. The Virtual Tape Server (VTS) emulates and presents only 3490E
models to the host. This is the only supported model and storage server.
Remove the entire code base for 3590/3592 models as it can be considered
dead code for quite some time already.

Teach the memory hotplug path to tear down KASAN shadow that
was mapped during early boot when a memory block is offlined.
Track for each sclp_mem whether its range was covered by the early
KASAN shadow via an early_shadow_mapped flag. When such a block is
deconfigured and removed via sclp_config_mem_store(), compute the
corresponding shadow range and call vmemmap_free() to unmap the
boot mapped shadow, then clear the flag.
Using vmemmap_free() for the early shadow is safe despite the use
of large mappings in the boot-time KASAN setup. The initial shadow
is mapped with 1M and 2G pages, where possible. The minimum hotplug
memory block size is 128M and always aligned (the identity mapping
is at least 2G aligned), which corresponds to a 16M chunk of at
least 1M aligned shadow. PMD-mapped 1M shadow pages therefore
never need splitting, and PUD-mapped 2G shadow pages can now be
split following the preceding changes.
Relax the modify_pagetable() sanity check in vmem so that, with
KASAN enabled, it may also operate on the KASAN shadow region in
addition to the 1:1 mapping and vmemmap area. This allows the KASAN
shadow unmapping to reuse the common vmem helpers.

Use scnprintf() instead of sprintf() for those cases where the destination
is an array and the size of the array is known at compile time.
This prevents theoretical buffer overflows, but also avoids that people
again and again spend time to figure out if the code is actually safe.

The KMSG_COMPONENT macro is a leftover of the s390 specific "kernel
message catalog" which never made it upstream.
Remove the macro in order to get rid of a pointless indirection. Replace
all users with the string it defines. In almost all cases this leads to a
simple replacement like this:
- #define KMSG_COMPONENT "appldata"
- #define pr_fmt(fmt) KMSG_COMPONENT ": " fmt
+ #define pr_fmt(fmt) "appldata: " fmt
Except for some special cases this is just mechanical/scripted work.

There shouldn't be any 31 bit code around anymore that matters.
Remove the compat layer support required to run 31 bit code.
Reason for removal is code simplification and reduced test effort.
Note that this comes without any deprecation warnings added to config
options, or kernel messages, since most likely those would be ignored
anyway.
If it turns out there is still a reason to keep the compat layer this
can be reverted at any time in the future.

For now, including <asm/pgalloc.h> instead of <linux/pgalloc.h> is
technically fine unless the .c file calls p*d_populate_kernel() helper
functions.
But it is a better practice to always include <linux/pgalloc.h>. Include
<linux/pgalloc.h> instead of <asm/pgalloc.h> outside arch/.

When the global kernel command line parameter
memory_hotplug.memmap_on_memory is set to false, per-memory-block
memmap_on_memory setting can still be set to true. However, when
configuring memory block, add_memory_resource() would configure it
without memmap_on_memory.
i.e.
Even if the MHP_MEMMAP_ON_MEMORY flag is set,
mhp_supports_memmap_on_memory() returns false unless the kernel command
line parameter "memory_hotplug.memmap_on_memory" is enabled. When both
the flag and the cmdline parameter are set, the memory block can be
configured with or without memmap_on_memory support.
To ensure consistent behavior, permit configuring per-memory-block
memmap_on_memory only when the memory_hotplug.memmap_on_memory kernel
command line parameter is enabled.
This is similar to commit 73954d379efd ("dax: add a sysfs knob to
control memmap_on_memory behavior")

The tape device type 3590/3592 and emulated 3490 VTS can handle a block
size of up to 256K bytes. Currently the tape device driver is limited to
a block size of 65535 bytes (64K-1). This limitation stems from the
maximum of 65535 bytes of data that can be transferred with one
Channel-Command Word (CCW).
To work around this limitation data chaining is used which uses several
CCW to transfer an entire 256K block of data. A single CCW holds a
maximum of 65535 bytes of data.
Set MAX_BLOCKSIZE to 262144 (= 256K) to allow for data transfers with
larger block sizes. The read_block() and write_block() discipline
functions calculate the number of CCWs required based on the IDAL buffer
array size that was created for a given block size. If there is more
than one CCW required for the data transfer, the new helper function
tape_ccw_dc_idal() is used to build the data chain accordingly.
The Interruption-Repsonse Block (irb) is added to the tape_request
struct so that the tapechar_read/write() functions can analyze what data
was read or written accordingly.

The tape device driver uses a single idal_buffer for I/O. While the
buffer itself can be arbitrary big, the limit for data transfer for a
single Channel-Command Word is at 65535 bytes (64K-1) since the count
field specifying the amount of data designated by the CCW is a 16-bit
unsigned value.
Provide functionality that allocates an array of multiple IDAL buffer
with the limitation mentioned above in mind.
A call to idal_buffer_array_alloc() allocates an array with a certain
amount of IDAL buffers which is determined based on the total size of
@size. Each individual buffer is limited to a size of CCW_MAX_BYTE_COUNT
(65535 bytes).
Add helper functions that determine the size (# of elements) and the
total data size covered by the array as well.
Current users of the single IDAL buffer are adapted to use the new
functions with one buffer to allocate.
The single IDAL buffer is removed from the tape_char_data struct.

Currently tapechar_check_idalbuffer() is part of tape_char.c and is used
to ensure the idal buffer is big enough for the requested I/O and
reallocates a new one if required. The same is done in tape_std.c when a
fixed block size is set using the mtsetblk command. This is essentially
duplicate code.
The allocation of the buffer that is required for I/O can be considered
core functionality. Move the idal buffer allocation to tape_core.c,
make it generally available, and reduce code duplication.

In contrast to all other helper functions used to build CCW chains,
tape_ccw_cc_idal() and tape_ccw_end_idal() return values using
post-increments, which results in returning the same CCW pointer.
Though, the intent of the CCW helper functions is to return the _next_
CCW in the chain, which can then be processed.
There is currently no actual issue, as tape_ccw_cc_idal() is not used
yet and tape_ccw_end_idal() is only used at the end of a chain.
Change both functions return statement to ccw + 1 and bring them in line
with the other helper functions.

The Read Opposite error recovery code required 2 extra CCWs to be
allocated in order to transform the request. As this error recovery code
for both 34xx and 3590 was removed the additional allocation isn't
required anymore. Reduce it to two.

On old native type 3590 tape devices a Read Opposite error recovery
procedure on Error Recovery Action Code (ERA) 26 was issued if a Read
Forward command failed. This recovery procedure was implemented with the
Read Backward command. This is no longer supported.
Remove 3590 ERA 26 and Read Backward related recovery code.

On old native type 3490 tape devices a Read Opposite error recovery
procedure on Error Recovery Action Code (ERA) 26 was issued if a Read
Forward command failed. This recovery procedure was implemented with the
Read Backward command.
As a preparation for a subsequent commit, that adds support for bigger
block sizes, remove the 34xx ERA 26 related recovery code. The recovery
code would need to be adapted to the bigger block sizes, without any
possibility to be tested, as modern Virtual Tape Servers (VTS) do
neither report ERA 26 on a Read Forward command failure nor support the
error recovery procedure anymore.

for standby memory blocks. This became critical in cases where there was
an imbalance between standby and online memory, potentially leading to
boot failures due to insufficient memory for metadata allocation.
To address this, memmap_on_memory was utilized on s390. However, in its
current form, it adds struct pages metadata at the start of each memory
block at the time of addition (only standby memory), and this
configuration is static. It cannot be changed at runtime (When the user
needs continuous physical memory).
Inorder to provide more flexibility to the user and overcome the above
limitation, add an option to dynamically configure and deconfigure
hotpluggable memory block with/without memmap_on_memory.
With the new interface, s390 will not add all possible hotplug memory in
advance, like before, to make it visible in sysfs for online/offline
actions. Instead, before memory block can be set online, it has to be
configured via a new interface in /sys/firmware/memory/memoryX/config,
which makes s390 similar to others. i.e. Adding of hotpluggable memory is
controlled by the user instead of adding it at boottime.

Patch 1 adds support for removal of boot-allocated memory blocks.
Patch 2 provides option to dynamically configure and deconfigure memory
with/without memmap_on_memory.
Patch 3 removes MHP_OFFLINE_INACCESSIBLE from s390. The mhp flag was
used to mark memory as not accessible until memory hotplug online phase
begins. However, with patch 2, it is no longer essential. Memory can be
brought to accessible state before adding memory, as the memory is added
during runttime now instead of boottime.
Patch 4 removes the MEM_PREPARE_ONLINE/MEM_FINISH_OFFLINE notifiers. It
is no longer needed. Memory can be brought to accessible state before
adding memory now, with runtime (de)configuration of memory.

Use scnprintf() instead of sprintf() for those cases where the destination
is an array and the size of the array is known at compile time.
This prevents theoretical buffer overflows, but also avoids that people
again and again spend time to figure out if the code is actually safe.

mhp_flag MHP_OFFLINE_INACCESSIBLE was used to mark memory as not
accessible until memory hotplug online phase begins.
Earlier, standby memory blocks were added upfront during boottime and
MHP_OFFLINE_INACCESSIBLE flag avoided page_init_poison() on memmap
during mhp addition phase.
However with dynamic runtime configuration of memory, standby memory can
be brought to accessible state before performing add_memory(). Hence,
remove MHP_OFFLINE_INACCESSIBLE.
